“Surgical steel” sounds like a specification. It is not one. There is no regulated standard behind the phrase, nothing a piece has to pass to carry it, and it gets printed on everything from properly certified 316LVM to unmarked alloy of unknown origin. It is the same trick as “waterproof” on a jewelry listing — an official-sounding word standing in for a number the seller would rather not give you.
The nickel problem
Surgical steel, in its honest form, means 316L or 316LVM stainless — an alloy of iron, chromium and nickel. The nickel is bound up in the alloy and for many people it never causes trouble, which is why steel remains perfectly reasonable jewelry. But nickel is the most common contact allergen there is, and a healing piercing is an open wound sitting against the metal continuously for weeks.
Implant-grade titanium — the ASTM F136 and F1295 designations, which unlike “surgical steel” are real specifications you can ask for by name — contains no nickel at all. That is the entire argument, and it is enough on its own for anything that is healing.
The weight problem, which nobody mentions
Titanium is roughly half the weight of steel for the same piece. On a lobe this is irrelevant. On cartilage it is not: a helix, conch, tragus or flat piercing is healing through tissue with a poor blood supply, and a heavier piece hanging in it moves more, tugs more and irritates more. The weight difference is a real part of why titanium is the professional standard for initial jewelry, and it is almost never the part that gets explained.
The timeline everyone gets wrong
This is the practical failure, more than material choice. A lobe heals in a couple of months. A cartilage piercing does not — it commonly takes six months to a year, sometimes longer, and it heals from the outside in, which means it looks finished long before it is. The irritation bumps people get months in are usually not infection and not a metal reaction. They are the result of changing the jewelry, sleeping on it or knocking it while the inside of the channel is still raw.
So the honest version of the material rule: implant-grade titanium for the entire healing period, and be pessimistic about when that ends. Once genuinely healed, solid gold at 14k, or properly certified steel if nickel has never bothered you, are both fine.
What to avoid outright
Plated anything in a fresh piercing, for the reason set out in the plating tiers: the coating is microns thick, it wears and it is porous, and what is underneath ends up against healing tissue. Gold under 14k in a healing piercing, because the alloy content rises as the karat falls and so does the chance of a reaction. Sterling silver anywhere in a piercing, because it tarnishes in contact with body fluids and can leave a permanent grey mark in the channel. And externally threaded jewelry, which is the one piece of purely mechanical advice here: the thread is on the shaft rather than the post, so you drag the screw thread through the healing channel every time you take it out. Internally threaded or threadless is smoother and worth asking for by name.
Buying, in one line
Ask for the standard, not the adjective. A seller who can tell you a piece is ASTM F136 titanium is a different kind of seller from one who says “surgical steel, hypoallergenic” — a word which, like waterproof, means whatever the person printing it wants it to mean.
